FAMILY SUPPORT UNDERTAKING
Republic of the Philippines — Voluntary family support · Template v0.2.0
This Family Support Undertaking (the "Undertaking") is made on September 26, 2026 at Quezon City, Philippines, by Roberto Villanueva, of legal age, residing at 9 Molave Drive, Tandang Sora, Quezon City 1116 (the "Giver"), in favor of Mateo Santos Villanueva, born on June 14, 2015 (the "Recipient"), the Giver's child, whose filiation is established by the child's PSA birth certificate, on which the Giver signed the acknowledgment of paternity. Payments are received by Maria Cristina Santos, residing at 45 Mabini Street, Quezon City 1100 (the "Payee").
1. Undertaking
The Giver voluntarily undertakes to give the Recipient support covering food and housing, medical and dental care, education, school fees, and supplies and transportation, on the terms of this Undertaking, in keeping with the duty of support between them under the Family Code (Articles 195 and 196).
2. Amount and payment
The Giver shall pay Twelve Thousand Pesos (PHP 12,000.00) every month to the Payee, on or before the 5th day, by Bank transfer to the payee's account ending in 4321.
The Giver shall also pay the following expenses directly: The giver pays the child's school tuition directly to the school each term.
3. Term
This Undertaking starts on October 1, 2026 and continues until the child turns 18 and completes a first college degree, whichever is later.
4. Review and adjustment
Support is given in proportion to the Giver's resources and the Recipient's needs. The Giver and the Payee shall review the amount at least every 12 months, and the amount may be increased or reduced when those resources or needs change. A change takes effect only when both agree in writing or a court orders it.
5. Records
Each payment shall be recorded through bank transfer records or a signed receipt, and each side shall keep them.
6. What this Undertaking does not do
This Undertaking is in addition to, and does not reduce, any support that the law requires the Giver to give. It does not waive or compromise the Recipient's right to future support, which the law does not allow (Civil Code, Article 2035).
This Undertaking does not decide custody, visitation, parental authority, legitimacy, or civil status.
If the Giver does not pay, the Recipient or the Payee may demand payment and may ask a court for support.
7. General terms
The Giver signs this Undertaking voluntarily. Philippine law governs it. If any provision is held invalid, the rest remains in force.
