What is a Child Travel Consent and Parental Authorization?

A Child Travel Consent and Parental Authorization is a written document in which one person gives another the authority to act, or to decide something, on their behalf in a stated matter.

What this template does. Prepare one child's domestic or international consent with current DSWD Travel Clearance Certificate, Certificate of Exemption, parental-authority, custody, itinerary, companion, support, safety, privacy, and notarial routing.

Also searched as

  • minor travel consent
  • DSWD travel authorization
  • parental consent to travel
  • affidavit of support and consent

Commonly used by

  • Parents
  • Guardians
  • Caregivers

Legal considerations

Formalities required. A private consent supports but never replaces a DSWD TCC or Certificate of Exemption, NACC/RACCO consent, court order, passport, visa, airline, immigration, destination, or current child-protection requirement.

How it is signed

Sworn before a notary public. You sign in front of a notary public, who checks your identity and has you swear to the statements (a jurat). Do not sign it beforehand.

What it draws on

  • Family Code
  • DSWD Memorandum Circular No. 22 s. 2024
  • Expanded Anti-Trafficking in Persons Act
  • Notarial Rules
  • Data Privacy Act

The exact provisions, with links to the official text, are listed under the Philippine law behind it.

How to complete your Child Travel Consent and Parental Authorization

The guided interview takes you through 8 steps. The document updates as you answer, and some questions appear only when your earlier answers make them relevant.

  1. Step 1

    Find the correct travel-authority route

A private consent is not a DSWD Travel Clearance Certificate, Certificate of Exemption, passport, visa, or custody order. Route the child's exact status first.

    Up to 10 questions
  2. Step 2

    Identify the child exactly

Match the PSA record, passport, and itinerary. Do not guess a civil-status fact or expose an unnecessary full registry number.

    Up to 10 questions
  3. Step 3

Record valid parental authority and consent

Married parents ordinarily consent jointly. For a non-marital child, parental authority ordinarily belongs to the mother unless a controlling court order establishes another route.

    Up to 18 questions
  4. Step 4

    Define one exact trip

Destination, purpose, dates, transit, lodging, host, and return plan must match the application, tickets, visa, invitation, and companion records.

    Up to 9 questions
  5. Step 5

    Verify the traveling companion

Authorize one identified person only. A change in companion can require a new DSWD application or certificate.

    Up to 10 questions
  6. Step 6

Set support, safety, and emergency authority

State who pays, what the companion may do, how the child stays in contact, and the limits of temporary authority.

    Up to 17 questions
  7. Step 7

    Build the current submission checklist

DSWD, NACC/RACCO, immigration, airlines, embassies, schools, events, and destinations can require different current evidence.

    Up to 5 questions
  8. Step 8

Prepare the accepted oath and jurat

DSWD's online travel-clearance process relies on the parents' participation in the online interview. A written consent is required when the companion files the application, a notarized oath of undertaking is required from a non-relative companion, and an affidavit of support is required for a child traveling alone. Airlines and immigration officers may still ask for a notarized consent, which this document supplies. The authorized notarial or consular officer—not the generator—completes identity, oath, venue, register, and certificate details.

    Up to 7 questions
