What is an Authorization Letter?

An Authorization Letter is a written document in which one person gives another the authority to act, or to decide something, on their behalf in a stated matter.

What this template does. Authorize one identified person to complete one limited administrative claim (including one identified payment), filing, pickup, follow-up, school, health-record, government, or records transaction with exact recipient, identity, custody, privacy, expiry, revocation, and handoff controls.

Legal considerations

Standard. Verify that the recipient accepts a letter. Use its mandatory form or the Special Power of Attorney generator for banking, real-property, borrowing, guaranty, contract, compromise, waiver, litigation, inheritance, partnership, gift, or another strict-dominion act.

How it is signed

Depends on the route you choose. How it must be signed depends on the choices you make in the questions. Your signing checklist states exactly which formalities apply to your version.

What it draws on

  • Civil Code on agency
  • Article 1878 special-power boundary
  • Recipient requirements
  • Data Privacy Act
  • Electronic Commerce Act

The exact provisions, with links to the official text, are listed under the Philippine law behind it.

How to complete your Authorization Letter

The guided interview takes you through 7 steps. The document updates as you answer, and some questions appear only when your earlier answers make them relevant.

  1. Step 1

Confirm a letter is the right instrument

A limited authorization letter works for a specific administrative transaction. Acts of strict dominion and many bank/property/court transactions need an SPA or prescribed form.

    Up to 8 questions
  2. Step 2

    Identify principal and representative

Use the exact legal names and only the minimum ID details the recipient actually requires.

    Up to 20 questions
  3. Step 3

    Describe one exact transaction

The representative should not have to guess—and the recipient should not be asked to infer authority from broad language.

    Up to 27 questions
  4. Step 4

    Make the boundaries explicit

A limited letter should say what the representative cannot do, especially where a third party might otherwise rely on ambiguous authority.

    Up to 4 questions
  5. Step 5

    Control records and personal data

An authorization helps identify the representative and purpose; it does not force unlawful disclosure or waive privacy rights.

    Up to 9 questions
  6. Step 6

    Limit duration and control revocation

Short, single-purpose authority is easier to verify and safer to revoke after completion or a lost document.

    Up to 9 questions
  7. Step 7

Execute and deliver the accepted package

Recipient requirements control. Notarization may be required by policy even when a simple signed letter would otherwise evidence authority.

    Up to 8 questions

I, Maria L. Santos, of 15 Sampaguita Street, Barangay Poblacion, Quezon City 1100, Philippines, authorize Ana M. Cruz, of 44 Rizal Avenue, Barangay San Lorenzo, Makati City 1226, Philippines, identified by Philippine driver's license reference ending 7719, to claim an identified item or delivery solely as stated below. Relationship: Adult sister. I am identified by Philippine passport reference ending 4821.

Appear at the Makati Central Claim Counter, present this exact signed letter and the listed accepted identity records, and claim the sealed package corresponding only to Tracking No. BEX-PH-2026-883190 and Claim Stub CS-55218.

Subject person/account/item: Package consigned to Maria L. Santos at the principal address, with no authorization concerning any other account, shipment, person, refund, or benefit. Transaction reference: Tracking No. BEX-PH-2026-883190 and Claim Stub CS-55218.

Documents/items: One sealed parcel weighing approximately 2.4 kilograms, sender shown as Luzon Books Trading, tracking BEX-PH-2026-883190, claim stub CS-55218, together with the recipient-issued release receipt; contents are not authorized to be opened or inspected by the representative.

Permitted necessary steps: Present this signed letter and listed ID copies, submit the exact application or claim stub, sign only a receiving or release log, inspect the item/document identity, and deliver it securely to the Principal. Location/channel: In person at Bayanihan Express Makati Central Claim Counter, 100 Ayala Avenue, during published claim hours and before the stated expiry date.

No fee-payment authority is granted.

Strict limits

The Authorized Person may perform only the exact administrative act and necessary listed steps; no ownership, contract, account-control, waiver, borrowing, substitution, onward disclosure, or unrelated discretion is granted.

This letter does not authorize the representative to sign or form a contract; borrow or obtain credit; sell, mortgage, lease, transfer, or encumber property; compromise, waive, settle, novate, arbitrate, litigate, or abandon a right; make a gift; enter a partnership; accept or repudiate an inheritance; bind the Principal as guarantor or surety; endorse an instrument; change account ownership or control; receive cash, checks, benefits, refunds, or proceeds; appoint a substitute; use data for another purpose; retain originals; or make an unstated certification, admission, guarantee, or factual representation.

Any signature by the Authorized Person is limited to a receipt, release log, filing acknowledgment, or transaction-status record that does not create or waive a substantive right or obligation. No password, PIN, OTP, biometric, private key, or unrestricted account credential is shared or authorized.

The Authorized Person must act within the exact scope and for the Principal's benefit, exercise ordinary diligence, follow lawful recipient instructions, obtain official receipts, protect items and records, disclose any conflict or incident, and stop and contact the Principal when an unexpected substantive decision or broader authority is requested.

Personal data and record custody

Minimum data: Principal and representative names, minimum ID references, verified contact details, shipment tracking and claim-stub references, and the transaction's release and handoff status. No sensitive personal information is intentionally included beyond what the named recipient lawfully requires.

Purpose and necessity: Verify the principal and authorized representative and release the one identified parcel because the Principal is outside Metro Manila during the claim period; no unrelated account, purchase, browsing, contents, or recipient data is necessary.

Authorized recipients: Assigned personnel of Bayanihan Express Makati Central Claim Counter who need the data for identity verification and release, Ana M. Cruz for the limited transaction, and Maria L. Santos for handoff and accounting. Original/copy handling: Use originals only for in-person verification where required; do not surrender an original unless expressly listed; mark and account for every copy, keep it secure, and prohibit unrelated reproduction or forwarding.

Completion handling: Return all originals, transaction output, receipts, unused copies, and funds immediately after completion; securely delete unnecessary working copies after confirmed handoff unless law or a documented recipient rule requires retention.

Incident process: Notify the Principal immediately of any loss, misdelivery, unauthorized access, suspicious request, credential request, or disclosure; stop the transaction where safe, preserve evidence, and follow the recipient's incident process.

This letter identifies authority and purpose; it does not compel the recipient to disclose data unlawfully, create a missing lawful basis, waive data-subject rights, authorize unrelated processing, or override confidentiality, privilege, professional duties, or the recipient's verification rules.

Before you start.

How do I make an authorization letter?
Write the date, your full name, the full name of the person you are authorizing, and the specific task, for example claiming a named document or one payment for you, then sign it. Offices often ask the representative to show a valid ID and a copy of yours, and some have their own form, so check the office's requirements before going.
Is an authorization letter the same as a special power of attorney?
No. An authorization letter is an informal note that is often enough for simple errands like picking up a document. Acts such as selling or mortgaging property, borrowing money, or making gifts need a special power of attorney (Civil Code Art. 1878), which is usually notarized.
Can I write an authorization letter in Tagalog?
Yes. Filipino and English are both official languages, and government offices are directed to use Filipino more in official communications. Our guide to authorization letters in Tagalog has free samples for PSA, a pickup, and one payment. The document made here is written in English.
How much is the Authorization Letter?
₱79, paid once. The interview and live preview are free, and the price covers the Word and PDF files, 30 days of revisions, and your account copy. There is no subscription. See all pricing →
How long does it take?
The guided interview has 7 steps and up to 85 questions. Some questions only appear when they apply to your situation, so you may answer fewer. You can read the document as it builds, and sign in to save a draft and finish later.
Do I need a notary?
Depends on the route you choose. How it must be signed depends on the choices you make in the questions. Your signing checklist states exactly which formalities apply to your version. Your signing checklist lists the exact steps for your version.
Is it reviewed by a lawyer?
This template’s current status is: Independent Philippine lawyer review pending. LAW.PH is legal self-help software, not a law firm, and will not describe a document as lawyer-reviewed until an independent Philippine lawyer has approved this exact version.
Is there a free template?
Yes. The sample on this page is free to read and to download as an editable Word file, with no sign-in. It uses fictional names, dates, and amounts, so replace every one. The paid version is built from your own answers, checks them as you go, and includes the signing checklist for your exact situation. Download the free sample (.docx)
Can I change it after I buy it?
Yes. For 30 days you can create a separate revision draft, update your answers, and download the revised files at no extra charge. The version you originally bought is preserved unchanged.
Which law does it follow?
It is drafted under Philippine law and relies on 8 official sources, each listed above with the provisions used and the date it was last checked.
