A Temporary Caregiver Authorization is a written document in which one person gives another the authority to act, or to decide something, on their behalf in a stated matter.

What this template does. Create a limited, time-bound care authorization with parental-authority, court-order, school, healthcare, emergency, local-transport, safeguarding, privacy, expense, recipient, revocation, and final-handoff controls.

Legal considerations

Heightened care. This delegates only specified practical care acts. It cannot transfer or renounce parental authority, create guardianship, decide custody, override a court order or recipient rules, or authorize international travel.

How it is signed

Depends on the route you choose. How it must be signed depends on the choices you make in the questions. Your signing checklist states exactly which formalities apply to your version.

What it draws on

  • Family Code
  • Child-protection laws
  • Emergency treatment law
  • Data Privacy Act
  • Electronic Commerce Act
  • Notarial Rules

The exact provisions, with links to the official text, are listed under the Philippine law behind it.

How to complete your Temporary Caregiver Authorization

The guided interview takes you through 7 steps. The document updates as you answer, and some questions appear only when your earlier answers make them relevant.

  1. Step 1

    Set the lawful care route

This authorizes limited practical care. It does not transfer parental authority, decide custody, or create guardianship.

    Up to 7 questions
  2. Step 2

    Identify every responsible person

Use complete legal names, reliable contacts, and only the minimum ID details the recipients require.

    Up to 23 questions
  3. Step 3

Limit the period, purpose, and places

A narrow, verifiable authorization is easier for schools, clinics, and other recipients to rely on.

    Up to 9 questions
  4. Step 4

    Choose only the needed permissions

Each selected permission appears as a bounded clause; everything else remains excluded.

    Up to 11 questions
  5. Step 5

Build the care and emergency plan

Give the caregiver usable facts and contacts without pretending a private letter can override a provider's lawful consent process.

    Up to 7 questions
  6. Step 6

Set safeguarding, money, and privacy controls

Record who may receive the child, how incidents are handled, and how child information and funds are protected.

    Up to 13 questions
  7. Step 7

Finish, deliver, and control the authorization

Recipient-specific school, clinic, activity, and identity rules still control acceptance.

    Up to 7 questions

Provide safe temporary care while both parents complete a short overseas work assignment and remain reachable for decisions reserved to them.

This authority begins September 5, 2026 and ends automatically at the end of October 5, 2026, or earlier upon completion, revocation actually communicated to the Caregiver and relevant recipient, a controlling order, or safe return of the Child. It does not renew by conduct.

School and release instructions: Ana may present the signed original and accepted ID, collect Sofia after classes, report absences, receive ordinary scheduling notices, and follow the school's current release log; she may not change enrollment or sign a new obligation. School: Mabini Learning Academy, 18 Commonwealth Avenue, Quezon City; Registrar +63 2 8555 3100.

Local movement: Direct local travel within Quezon City and Pasig City for the listed residence, school, clinic, and approved activities using a seat-belt-equipped private car or licensed ride service with direct adult supervision.

The Caregiver may complete a recipient's verification or acknowledgment only when it records the actual care task and does not create a contract, waive a right, make a legal declaration for the Child, or expand this authority.

3. Health and emergency plan

Known allergies, conditions, dietary needs, and warning signs: Peanut allergy requiring avoidance and prompt response to symptoms; mild asthma managed under the current written pediatric care plan; no other known condition.

Medication plan: Keep the prescribed inhaler and epinephrine auto-injector in their original labeled containers, follow the attached physician instructions, record every use, and make no dose or medicine change.

Nothing in this document requires a hospital or clinic to delay appropriate initial treatment and support in an emergency or serious case. It does not authorize elective, invasive, experimental, irreversible, or other major treatment; change a prescription; waive a provider's lawful consent process; release a provider from liability; or make the Caregiver the Child's legal guardian or general medical decision-maker.
