To: Andrea M. Reyes, People Operations Director Bayanihan Digital Services, Inc. 18/F One Ayala Tower, Makati City 1226
Subject: Written Notice of Resignation
Dear Andrea M. Reyes:
1. Notice and intended effectivity
I, Juan Miguel Santos, voluntarily give written notice of my resignation from my position as Senior Customer Support Specialist in Customer Experience. My intended last working day is September 30, 2026, which is at least thirty (30) days (one month) after delivery of this notice on August 31, 2026, as Article 300(a) [formerly 285(a)] of the Labor Code, as renumbered, requires.
My brief voluntary reason is: I have decided to pursue a new professional opportunity.
2. Employment record and voluntary intent
My employment began on January 10, 2022. My employee reference is EMP-1042. This signed notice is my formal and voluntary act of relinquishing the position on the date stated above.
No unsigned or blank form, later employer notation, clearance document, or separate quitclaim may be treated as part of this notice unless I knowingly sign that separate completed record.
3. Turnover and company property
I will prepare a written handover of active customer escalations, deadlines, records, counterparties, and pending decisions and coordinate it with the designated team lead.
I will coordinate the handover with Maria L. Cruz, Customer Experience Team Lead.
I will continue performing lawful assigned duties and coordinate any approved leave during the notice period.
Company property, records, funds, keys, or accounts in my custody are: Laptop asset BD-991, access card AC-1042, headset, and assigned security token, to be returned against a signed inventory receipt. I propose to return or account for them through the documented handover process.
I will return company records and devices, transfer authorized business files, surrender or close company access, and retain no unauthorized company copies.
4. Final pay and employment records
Please provide an itemized computation and release of all final pay due within 30 calendar days from separation, unless a more favorable company policy, individual agreement, or collective agreement applies, including the components supported by payroll records, law, contract, CBA, policy, or established practice. This request is not my acceptance of an unknown or disputed computation.
I request a Certificate of Employment for general lawful employment-record purposes. Please issue it within three days from this request and keep its contents accurate and appropriately limited.
I also request the following ordinary separation records when due: BIR Form 2316 and the final payroll, benefits, and clearance records ordinarily provided. BIR Form 2316 is due on the day the last payment of wages is made (National Internal Revenue Code, Sec. 83(A)).
5. Delivery, contact, and rights preserved
This notice will be delivered through the documented channels identified below as follows: Signed original delivered to the People Operations Director at the principal office, with an exact PDF sent to the official company email address. Please acknowledge receipt without changing the notice date or the text of my signed copy.
After separation, records and lawful communications may be sent through: juan.santos@example.com or +63 917 555 0123.
This letter is not a clearance, release, quitclaim, settlement, admission of liability, final-pay computation, authorization for an unlawful deduction, or waiver of a mandatory right. Each separate document must be complete, accurate, supported, and considered on its own terms.
6. Closing
Please confirm receipt and identify the authorized contacts for handover, property return, final-pay computation, and employment records.
Respectfully submitted,
